What are Hyaluronic Acid Fillers and Biostimulatory Fillers?

The current market for dermal filler therapy is dominated by a single class of dermal fillers called hyaluronic acid fillers. These injectable gels are a mainstay of nonsurgical facial rejuvenation and beautification. They last around one year, are safe and reliable, and offer meaningful and reproducible results. These biomaterials exert their effect through two means:

Degree of hydrophilicity: Hydrophilicity refers to how much a gel will “attract water.” Picture a sponge. Some sponges can hold more water than others. Similarly, some injectable gels hold more water than others.

Degree of rigidity: This characteristic shows how strong and rigid the gel will be. The stronger the gel, the longer it lasts, but it's also more firm and palpable. So there is a takeaway there: nobody wants rigid bumps or shapes sticking out of their facial skin, but everyone wants their filler to last longer than it does, but not if it means giving up a natural appearance.

Within each of these brands are multiple types of hyaluronic acid fillers, and each filler has specific properties that make it firmer, softer, or better at absorbing water. These characteristics can be capitalized on by your injector to create your best, most natural result. As such, there is no “best” hyaluronic acid filler. They all have points of differentiation, and if understood by your injector, these points of differentiation can be used to elevate your results.

Hyaluronic acid fillers have literally maintained nearly 100 percent market share in the filler arena since they were introduced in 2003. However, over the last three years, the market share of hyaluronic acid fillers has fallen precipitously, and they now only represent around 75 percent of the market. They are largely being outcompeted by a more advanced class of dermal fillers known as bio-inductive dermal fillers.

What are Biostimulatory Fillers?

Bioinduction refers to a process where a drug or implant interacts with a patient’s own body, and then allows the patient’s own body to fix itself. In other words, the filler acts as an “on switch” to the human body after injection. The filler turns on the “on switch,” and then the body starts to fix itself. The bioinductive fillers, due to these unique characteristics, do so much more for patients than their hyaluronic acid filler counterparts.

They are structural, tend to lift as much as they fill, and tend to last markedly longer than their hyaluronic acid counterparts.

These fillers are usually deposited into deeper, more structural areas of the face and correlate with bony prominences such as the jawline and cheeks. Examples of bioinduction fillers include:

Radiesse, Radiesse (+)®

This specific filler is our workhorse filler. It offers the production of type I collagen (young collagen), provides the greatest degree of lift on the market, and lasts up to two years after injection (twice as long as all hyaluronics). It is FDA approved for the cheek and jawline.

Sculptra Aesthetic®

This specific filler works through an inflammatory pathway, providing type III collagen. It is FDA approved for HIV-induced lipoatrophy and cheek wrinkles. We use this filler for body applications and scar revisions.

Bellafill®

Bellafill is often called a “permanent filler” because the body doesn’t break it down like hyaluronic acid fillers. It’s made from polymethyl methacrylate (PMMA), a biocompatible substance used in medical implants and other long-term applications. These tiny microspheres are suspended in collagen, giving immediate volume while also supporting long-term tissue structure.

Bioinduction or Hyaluronic Acids?

Neither is better than the other. In fact, the two synergize with each other and are best when used in combination.
